The Health Services Director (LPN) will be an experienced, dynamic leader, with superb communication skills and a passion to serve. The Health Services Director serves as a liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide quality care to our assisted living residents. Additionally, this role is responsible for hiring, training and managing a staff that is capable of providing superior care, while maintaining the physical and emotional health of residents, and in accordance with current federal, state and local standards, guidelines and regulations.

 

Responsibilities

 

  • Conduct and coordinate the potential assisted living resident assessment program in accordance with current rules, regulations, and community policies and procedures that govern resident assessment including:
    • Ensure that an initial assisted living resident assessment is completed prior to each resident’s move-in date and a comprehensive care plan is completed upon move-in
  • Conduct routinely scheduled re-assessments on each assisted living resident, and change of condition assessments as needed to:
    • Ensure each assisted living resident is receiving the best possible care and programs to meet their individual needs
    • Conduct interviews and observations of the assisted living residents on a continual basis
    • Alert Executive Director, family members and others (as necessary) of any changes in the physical or emotional health of the assisted living resident, and the department’s ability to meet those needs
  • Ensure continuity of the assisted living residents’ total care regimen by developing methods for the coordination of nursing services with other resident services (20%)
    • Work closely with all departments in the community to ensure assisted living residents are receiving all available services to enhance their quality of life
    • Act as a resource and care manager for the assisted living residents and their families
    • Ensure that the calendar is made and followed monthly to ensure that our assisted living residents are engaged in meaningful activities daily
  • Recruit, interview, hire, manage, schedule, motivate, evaluate and supervise department staff in accordance with community policies, procedures and established budgets
  • Perform all administrative duties accurately and timely including team member evaluations, necessary forms, reports, evaluations, studies, surveys, resident records, assessments, care profiles, etc.
  • Coordinate prescription orders with doctors’ offices, as well as pharmacy delivery of medications
  • Responsible for the overall safety and health of all assisted living residents
  • Aid in the community marketing efforts through positive interactions, acting as a liaison between the community, and families/outside health service providers
  • Manage the department within the budgetary guidelines, track inventory, and order supplies as needed
  • Ensure regulatory compliance and report any issues or concerns immediately
  • Comply with Clearwater Living standards and regulations to encourage safe and efficient community operations
  • Represent the Clearwater Living principles and core values on a daily basis
  • Perform other duties and tasks as assigned or required

Qualifications

  • Minimum five to seven years of experience in senior living, healthcare, or related industry
  • Completion of required specialized Nursing coursework for certification or certification as an RN, LPN or LVN
  • Minimum two to three years of supervisory experience to include training and supervising staff and developing and managing a budget
  • First Aid/CPR/BLS Certification required
  • Background clearances as required by government regulations
  • Must meet health requirements, including TB
